520 Athletics is a trading name of 520 RVA LLC. In this agreement, “520 Athletics” means 520 RVA LLC.

Read the agreement below before you book. When you go to book a session, a parent or legal guardian is required to sign this waiver on the booking page before the booking is confirmed. There is nothing to sign on this page.

Assumption of risk

Strength, speed, and sports performance training involves inherent risk of physical injury, including but not limited to strains, sprains, fractures, concussion, heat illness, cardiac events, and in rare cases permanent disability or death. These risks cannot be eliminated by instruction, supervision, or equipment.

The undersigned has had the opportunity to ask questions about the program, understands these risks, and voluntarily accepts them on behalf of the athlete.

Covenant not to sue for inherent risks

The undersigned agrees not to bring a claim against 520 RVA LLC, its owners, coaches, employees, contractors, or facility landlords for injury, illness, or loss arising from the inherent risks of athletic training described in section 01 — that is, risks that remain after reasonable care has been taken.

This does not waive any claim arising from negligence, gross negligence, or willful misconduct by 520 Athletics or its staff. Those rights are expressly reserved, and nothing in this agreement limits them.

Fitness to participate

The undersigned represents that the athlete is in good health and physically able to participate, and that any condition, injury, allergy, or medication that could affect safe participation has been disclosed to 520 Athletics in writing.

Emergency medical authorization

If the athlete is injured and a parent or guardian cannot be reached, the undersigned authorizes 520 Athletics staff to summon emergency medical services and consent to examination and treatment, and accepts responsibility for the cost of that care.

Facility rules and conduct

The athlete agrees to follow coaching instruction, use equipment as directed, and refrain from conduct that endangers themselves or others. 520 Athletics may end participation for conduct or safety reasons.

Required training apparel

Athletes are required to purchase and wear 520 Athletics apparel during all training sessions. Apparel is available through the online store or at the facility. Athletes must also wear appropriate athletic footwear; turf shoes are required for field work.

An athlete may train in their own athletic clothing for a first session while apparel is ordered. If cost is a barrier, contact Adam before the first session — no athlete is turned away over apparel.

Media release

The undersigned grants 520 RVA LLC permission to photograph and record video of the athlete during training, and to use those images and recordings in promotional and marketing material, including the website, social media, and printed material.

The athlete also consents to this use. No payment is due for it. Consent may be withdrawn at any time in writing, after which 520 Athletics will stop future use of the athlete's likeness; material already printed or published cannot always be recalled.

Governing law

This agreement is governed by the laws of the Commonwealth of Virginia. If any provision is held unenforceable, the remaining provisions stay in effect. This agreement remains in force for all sessions until revoked in writing.
